近江連合と東城会の抗争事件から数日後。桐生一馬は狭山薫と別れ、沖縄で養護施設を営むため澤村遥とともに東京を発つ。そして養護施設アサガオで孤児の子供たち8人と平穏な日々を送っていた。しかし1年後、沖縄にある問題が押し寄せる。
米軍基地増加計画とリゾート開発計画。それに関わる東城会と二人の政治家の存在。市民の反対が増加する中、アサガオにも土地買収の問題が発生する。土地の所有者である地元極道組織・琉道一家とのいざこざ。それが徐々に国家規模の計画と繋がっていく。しかし、大吾の力もあって事態はひとまず落ち着き、桐生たちにもまた平和が戻ってくる。
さらに1年後の2009年3月、琉道一家組長・名嘉原茂と、東城会六代目会長・堂島大吾が謎の男に銃撃され倒れる。これにより止まっていた計画が再び動き出す。そしてその犯人が風間新太郎にそっくりだったことに衝撃を覚える桐生。そして頭を失った東城会では跡目と桐生復帰をめぐって幹部たちが抗争を繰り広げ始めていた。桐生は謎の男と事件の真相を確かめるため、そして沖縄やアサガオのため、一人で巨大な陰謀に立ち向かって行く。

A few days after the incident between Omi Rengo and Tojokai. Kazuma Kiryu left Tokyo with Haruka Sawamura to run an orphanage in Okinawa. They were living a peaceful life with eight orphaned children at Asagao, an orphanage. A year later, however, Okinawa is beset with problems.
A plan to increase the number of U.S. military bases and a resort development project. The Tojo Kai and two politicians are involved. As opposition from citizens increases, Morning Glory also faces the problem of land acquisition. The land is owned by the Ryudo family, a local political organization, and the two sides get into an altercation. Gradually, this leads to a national-scale plan. However, with Daigo’s help, the situation is temporarily calmed down and peace returns to Kiryu and his family.
One year later, in March 2009, Shigeru Nagahara, head of the Ryudo clan, and Daigo Dojima, the sixth president of the Tojokai, are shot and killed by a mysterious man. This puts the halted plan back in motion. Kiryu is shocked to find out that the murderer looks exactly like Shintaro Kazama. The executives of the Tojokai, who have lost their heads, begin to fight over the heir and Kiryu’s return to the Tojokai. Kiryu, in order to find out the truth about the mysterious man and the case, and for the sake of Okinawa and Morning Glory, goes alone to confront the huge conspiracy.
