I first wondered why his eyes were drawn that way, then realized that he's a kid who've been through a lot of harshness in different ways. Somehow, he's more mature and less of a 'kid', his childhood seems to be projecting towards practicality rather than being free which all kids should get.
I think the eyes imply that he's been through a lot and it distorted his perception about himself. He must be who he'd want to have and show it to others, because that's what he wished he has. It's like savior complex.

I think the reason why Kotaro didn’t want to go home is because he knows that not ALL kids get educated and he wants to make sure he doesn’t waste his school days.
In kotaros view getting a fever while being homeless and starving would be a death warrant knowing you dont have anyone to take care of you especially if you are young.
